3. Power of Attorney Copy Certification: download form (CA Prob. Code § 4307)

4. Request For a Copy of a Notary Journal Line Item: download form (CA Gov. Code §§ 8206[c][e] and 8206.5)

   Download, pre-fill notary’s name, mailing and email addresses. The requester will take care of the rest of the form.

5. Witness Affidavit for Signature by Mark: download form (CA Civ. Code § 14[a])
